Paracetamol dose for a child by weight

The calculator works out a child’s paracetamol dose by weight — as stated in the leaflet (10–15 mg/kg per dose). Enter weight and syrup concentration to get the single dose in mg and mL, the daily maximum and the interval. It’s a reference guide per the official leaflet, not a prescription: a doctor confirms the dose and whether it’s appropriate.

How the dose is calculated

Paracetamol for children is dosed by weight, not age: 10–15 mg per kilogram per dose. For a 15 kg child that’s about 150–225 mg. Per day — no more than 60 mg/kg and no more than 4 doses.

Dosing by weight is more accurate than “by age from the box”, because children of the same age vary a lot in weight.

How to measure the syrup

The bottle states the concentration — how many milligrams per 5 mL (e.g. 120 mg/5 mL or 250 mg/5 mL). Enter it and the calculator converts the dose from milligrams into millilitres of syrup.

Measure only with the syringe or measuring cap from the pack, not a “teaspoon”. Suppositories and tablets are dosed by weight too, but choose the form with a doctor.

What not to do

Don’t exceed the single dose or daily maximum, shorten the interval, or give paracetamol together with other products that also contain it — overdose is dangerous for the liver.

See a doctor urgently: a child under 3 months, fever lasting over 3 days, lethargy, rash, seizures, refusal to drink or other warning signs. Fever is a symptom; a doctor assesses the cause.

Frequently asked questions

  • By weight: 10–15 mg per kilogram per dose, up to 60 mg/kg a day and no more than 4 doses. The calculator gives the single dose in mg and mL of syrup from weight and concentration.

  • It depends on the bottle’s concentration (mg per 5 mL). Enter it and the calculator converts the dose into millilitres. Measure with the syringe or cap from the pack.

  • No more often than every 4–6 hours and no more than 4 doses a day. Exceeding the dose or shortening the interval is dangerous for the liver.

  • Usually from 3 months; younger only on a doctor’s advice. Any fever in a child under 3 months needs a doctor right away.

  • If fever lasts over 3 days, spikes very high or comes with lethargy, rash, seizures or refusal to drink — see a doctor. Antipyretics relieve the symptom but don’t treat the cause.
