Why calcium is corrected for albumin

Blood calcium comes in two forms: protein-bound (mostly to albumin) and free (ionized) — the biologically active one. A routine test measures total calcium, which is affected by the albumin level.

With low albumin (e.g. in liver or kidney disease, malnutrition) total calcium reads falsely low. The correction returns the estimate to what it would be at a normal albumin.

Formula and units

Payne’s correction in SI units: corrected calcium = measured + 0.02 × (40 − albumin in g/L). In conventional units: + 0.8 × (4 − albumin in g/dL). The calculator uses the unit system you choose.

The correction is an approximation and isn’t perfect (especially in kidney failure). When accuracy matters, a doctor orders a direct ionized calcium measurement.

What deviations mean

Persistently high corrected calcium (hypercalcemia) is most often linked to hyperparathyroidism or cancer; low (hypocalcemia) to vitamin D deficiency, hypoparathyroidism or kidney disease.

A single value isn’t a diagnosis. Calcium is assessed together with albumin, parathyroid hormone, vitamin D and phosphate, and a doctor interprets it.
