The lymphocytes line frightens people more than almost any other, because the word "leukaemia" appears next to it in every search box. In reality the overwhelming majority of shifts in this line are explained by a virus caught a fortnight ago, or simply by the age of a child. Normal lymphocyte counts in adults and children

Lymphocytes are the cells of antiviral and antitumour immunity. Their share depends heavily on age, and that is the main source of false alarm for parents.

Absolute lymphocyte count: why the percentage misleads

Most forms print lymphocytes twice: as a percentage of all white cells, and as an absolute number (×10⁹/L). The second one carries the clinical meaning.

A percentage is a fraction with a denominator. If someone has had a bacterial infection and their neutrophil number doubled, the lymphocyte share falls even though no lymphocytes were lost. The reverse also happens: when neutrophils drop, the lymphocyte percentage jumps to 60–70% with a perfectly normal absolute count. This is relative lymphocytosis — arithmetic of the white cell differential, not disease.

  • lymphocytosis — an absolute count above 4.0–4.8 ×10⁹/L;
  • lymphopenia — below 1.0 ×10⁹/L;

High lymphocytes: what causes lymphocytosis

Lymphocytosis is an immune response, not a disease in itself. The causes are distributed very unevenly: infection accounts for the great majority of cases.

Viral infection. The commonest cause by far — respiratory viruses, influenza, chickenpox, viral hepatitis, cytomegalovirus. High lymphocytes appear in the second week and settle 2–6 weeks after recovery, which is why a repeat count is worth doing no sooner than a month later.

Infectious mononucleosis. A distinctive picture: Epstein-Barr virus produces marked lymphocytosis with atypical mononuclear cells on the film, enlarged nodes, sore throat and an enlarged spleen — described in detail in infectious mononucleosis.

Whooping cough. In children and unvaccinated adults it causes a characteristically high lymphocyte count alongside paroxysmal coughing — see whooping cough.

Chronic infection. Tuberculosis, brucellosis, toxoplasmosis and syphilis give a moderate but persistent rise.

Stress lymphocytosis. Trauma, surgery, myocardial infarction, a seizure or heavy exertion release lymphocytes into the circulation for a few hours. The picture then returns to baseline.

Smoking and splenectomy. These produce a moderate, stable elevation without acute symptoms. In thyrotoxicosis the lymphocytosis is usually relative — the neutrophil share falls rather than the lymphocytes rising.

Chronic lymphocytic leukaemia. The most serious and, in the young, the rarest of the causes. It is a disease of older people, with a median age at diagnosis around 70. It is suspected when an adult has a persistent count above 5.0 ×10⁹/L for months with no infection to explain it. The diagnosis is made by flow cytometry, not by a blood count.

High lymphocytes in children are usually age, not disease

From a few months of age until 4–5 years lymphocytes normally outnumber neutrophils — physiological lymphocytosis. A two-year-old with 65% lymphocytes is entirely healthy, though the same figure in an adult would need explaining.

Two age-related crossovers explain nearly every parental worry: the first at about day 4–5 of life, when lymphocytes first overtake neutrophils, and the second at 4–5 years, when neutrophils move ahead again. Between those points the differential is lymphocyte-dominated by definition.

Low lymphocytes: causes of lymphopenia

Lymphopenia means an absolute count below 1.0 ×10⁹/L in an adult. Unlike lymphocytosis it is less often an incidental finding and more often needs an explanation.

  • Severe acute infection and sepsis. Lymphocytes migrate into tissues, so few remain in the blood at the height of illness. This is temporary.
  • Glucocorticoids. Prednisolone and similar drugs lower lymphocytes within hours. This is an expected pharmacological effect, not a complication.
  • Chemotherapy and radiotherapy. Direct marrow suppression; recovery takes months.
  • HIV infection. Progressive loss of CD4 lymphocytes is characteristic of the condition coded as HIV disease B20. Persistent unexplained lymphopenia in an adult is a direct indication for an HIV test — routine, not dramatic.
  • Autoimmune disease. Systemic lupus erythematosus, rheumatoid arthritis, sarcoidosis.
  • Protein and renal losses. Nephrotic syndrome, protein-losing enteropathy, severe malnutrition.
  • Inherited immunodeficiency. Presents in childhood with repeated severe infections.

Red flags: when lymphocytosis needs a haematologist

Plainly, without softening — these combinations should not wait:

  • lymphocytosis persisting beyond 3 months in an adult with no infection to explain it;
  • an absolute count above 5.0 ×10⁹/L with no acute symptoms;
  • lymphocytosis with painless enlarged nodes, especially firm and matted ones;
  • weight loss over 10% in six months, night sweats, fever without infection;
  • lymphocytosis together with falling haemoglobin and platelets — a sign that normal marrow output is being displaced;
  • lymphopenia below 0.5 ×10⁹/L confirmed on a repeat sample;
  • an enlarged spleen or liver on examination.

One such feature warrants a planned visit; two or more warrant not waiting.

What to do with an abnormal lymphocyte result

  1. Find the absolute figure on the form. If it is missing, calculate it with the formula above.
  2. Recall the previous 4–6 weeks: infection, fever, steroid courses.
  3. Look at the other lines. A change in lymphocytes alone and a change across the whole differential are different situations.
  4. Repeat the count in 4–6 weeks if you feel well. A single reading means little; the trend means a great deal.
  5. Bring both forms to the appointment, so the doctor sees a trajectory rather than a dot.

Which tests clarify the picture

The next step is the whole differential rather than one number: neutrophils cover the bacterial response, monocytes chronic inflammation and recovery, eosinophils allergy and parasites. When lymphopenia comes with anaemia, iron stores and vitamins are assessed too — the panel is listed under anaemia testing.

Depending on findings a doctor adds a manual blood film (atypical mononuclear cells, blasts), Epstein-Barr and cytomegalovirus serology, an HIV test, and — for persistent adult lymphocytosis — flow cytometry. That last test, not the blood count, is what separates reactive lymphocytosis from a clonal one.
